These forms are placed on the website for your convenience. Please download the forms you need, fill them out and mail or bring them to the Township Office.
Please read the instructions for each permit so that your application package is complete. Forms are presented in Microsoft Word and/or Adobe PDF formats. For most applications drawings and/or plans are required. Permit fees are established by resolution by the Board of Supervisors and will be calculated by the permit officer after reviewing the submission.
All forms are available at the Township office and can be mailed or e-mailed to the applicant upon request.
The Building Code Official and Inspector is an in-house employee of Damascus Township: Bob Bates Inspections Honesdale, PA 570.493.1716
All inspections must be performed by Bob Bates or his designee or no Certificate of Occupancy will be issued. Other third-party inspections are allowed, but all inspections ust be observed or accompanied by Bob Bates, and all fees established by the Township will be due.
ALL permits must be obtained through the Township Office and ALL fees be paid to the Township by check or money order only. Cash and cards are not accepted.

The Sewage Enforcement Officer (SEO) for Damascus Township is Chris Martin.All inquiries regarding septic systems and perc testing should be directed to him. He is only available during the following hours.
Thursday 9am -12pm at Damascus Township: 570.224.4410
Wednesday 9am -12pm at Paupack Township: 570.226.0442
Sewage applications must be used to apply for a permit and to be used to soil test your property.
The Applicant is to only complete PART I. APPLICANT AND SITE INFORMATION of the application, sign and date at the bottom, then return to Damascus Township with the appropriate fees.

Request Assistance from Wayne County Conservation District
Regardless of the size and scope of the project, every contractor on site must provide proof of Workers Compensation Insurance prior to entering onto the site. Any worker on the site not covered by Workers Compensation Insurance will be cause to immediately shut the job down and could result in revocation of any or all permits.

Workers' Compensation
Workers’ Compensation Insurance Affidavit (Required for UCC applications where contractor does not carry Workers’ Compensation Insurance, and for homeowners doing their own work). This form must be notarized.
