Verbal complaints and incomplete forms will not be investigated. The complaint
may be anonymous, however, anonymous or non-specific complaints will not be
investigated. Further information may be needed, and without the ability to
contact the complainant may result in no investigation. Complaints are not
considered public record, are not subject to a Right To Know request, and the complainant's name will not be revealed to
anyone without a court order. Additional sheets and photos may be attached.
