These forms are placed on the website for your convenience - just download
the forms you need, fill out the forms and mail or bring them to the Township
Office. Please read the instructions for each permit so that your application
package is complete. Forms are presented in Microsoft Word and/or Adobe PDF
formats. Word forms are interactive and can be filled out on your computer and
then printed. You'll need to print the PDF forms and fill them out by hand. For
most applications drawings and/or plans are required. Permit fees are
established by resolution by the Board of Supervisors and will be calculated by
the permit officer after reviewing the submission.
All forms are available at the Township office and can be mailed to the
applicant upon request.

ALL permits must be obtained through the Township Office.
Bob Bates is the Building Code Official and the designated Building Code
Inspector for the Township. As such, all inspections must be performed by him or
his designee or no Certificate of Occupancy will be issued. Other third-party
inspections are allowed, but all inspections must be observed or accompanied by
Bob Bates.

CABIN EXCLUSION FORM (pdf) A Zoning Permit IS required and all Zoning
requirements must be met, but this type of application excludes your cabin from
UCC requlations, except for the presence of smoke detectors, carbon monoxide
detectors, and fire extinguishers. Please read this form carefully and consider
the implications of no construction inspections. There will be no construction
inspections, and no Certificate of Occupancy will be issued. The Township will
assume exactly no liability for construction methods and materials
for a recreational cabin. The Township will inspect only for smoke alarms,
carbon monoxide detectors and fire extinguishers after the structure is
completed. A Zoning Use Certificate and Certificate of Compliance for
Recreational Cabins will be issued following an inspection for Zoning
requirements and for the presence of smoke and carbon monoxide detectors and
fire extinguishers as of the date of the inspection.
